PatientsForce Companion: Postoperative readmission rates correlated with daily step count.

According to recent studies published in international journals, it has been confirmed that a decrease in postoperative walking distance among patients is positively correlated with readmission rates. This study, which collected data from 215 patients with an average age of 63 years, utilized post-discharge mobile device monitoring to accurately predict the likelihood of patients being readmitted following surgery. Specifically, if the daily walking distance (step count) decreased by more than half for two consecutive days after discharge, the probability of readmission was significantly higher. The accuracy of this predictive method is approximately 90%, highlighting the importance of monitoring patients' daily activity levels for post-discharge care.

Ding Yuzhi, Senior Nursing Manager of the former Northern Rong Cardiovascular Department, stated that continuous care for patients after discharge is currently a pain point in medical care. Despite patients and their families striving to follow the guidance provided at discharge, the lack of continuous monitoring and professional advice makes the recovery journey uncertain for most patients.

Addressing this need, the Care Companion Cardiovascular Care Program by the PatientsForce Integrated Healthcare Group offers a solution. This program incorporates the use of Garmin's international original health monitoring smartwatch, integrating the data into a digital care platform. Professional nurses monitor the indicator data collected from the health wearable device to provide patients with personalized recovery plans and lifestyle recommendations. This personalized care helps improve patients' recovery efficiency and enhances their confidence. Additionally, with the support of remote care services from medical professionals, patients can easily consult or schedule appointments with a professional team including physicians, pharmacists, nutritionists, and rehabilitation therapists through the LINE@ messaging app.

Nurse Ding Yuzhi stated that this innovative home care model not only provides patients and their families with more reassuring and efficient recovery companion care but also provides medical professionals with a new tool to realize continuous recovery management plans. This model also improves patient compliance with medical advice, extending continuous care from hospitals to individuals, providing affordable and clearly defined care services. Through closer integration with healthcare institutions, the model achieves patient-centered care goals.
